Qualifications:

• Graduate of an accredited Nursing program
• Collaborative, interdisciplinary approach
• Commitment to excellent patient relationships and communication

Job Summary:

• Assessment/evaluation of patients needs/deficits/strengths
• Creation and execution of treatment plan
• Education to client and support system of interventions and treatment goals
• Documentation that follows reimbursement guidelines
• Adequate coordination of care/services among multi-disciplinary team members/physicians
• Assisting responsibility in Business Development & Sales for the agnecy
• Counseling of clinical staff
